Advanced Clinical Practitioner (ACP) Home Visiting Service

\nAt IC24, were looking for an Advanced Clinical Practitioner (ACP) to join our East Sussex Home Visiting Service. This is an opportunity to work at the forefront of community-based care, delivering high-quality clinical assessments and treatment to patients in their own homes while supporting wider urgent and primary care services.
\nAs an experienced clinician, you'll work autonomously within your professional scope of practice, providing assessment, diagnosis, treatment and prescribing for patients presenting with acute, urgent and undifferentiated conditions. You'll play a key role in helping patients receive the right care in the most appropriate setting, reducing unnecessary hospital admissions and improving patient outcomes.
\nWhat you'll focus on
\n? Delivering advanced clinical care in the community
\nYou'll assess, diagnose and manage patients in their own homes, providing high-quality clinical care for a wide range of urgent and primary care presentations.
\n? Making safe, evidence-based decisions
\nYou'll use advanced clinical reasoning skills to determine appropriate treatment plans, referrals, investigations and follow-up care while managing clinical risk effectively.
\n? Independent prescribing
\nAs an Independent Prescriber, you'll prescribe medication following clinical assessment and examination, ensuring safe and effective patient-centred care.
\n? Supporting continuity of care
\nYou'll work closely with GPs, nurses, allied health professionals, NHS 111, community services and secondary care partners to ensure patients receive coordinated and seamless care.
\n? Contributing to service improvement
\nYou'll support audits, clinical governance activities, shared learning and quality improvement initiatives that enhance patient outcomes and service delivery.
\n? Developing clinical excellence
\nYou'll maintain your continuing professional development, contribute to mentoring and shared learning, and help promote best practice across the wider organisation.
\nThis role forms part of our East Sussex Home Visiting Service, supporting patients across the community. You'll work as part of a highly skilled multidisciplinary team delivering responsive care where it is needed most, helping patients avoid unnecessary hospital attendance wherever appropriate.

\nIs confident assessing and managing urgent and undifferentiated presentations
\nHas strong clinical reasoning and risk management skills
\nCommunicates effectively with patients, carers and healthcare professionals
\nEnjoys delivering care within patients' homes and community settings
\nTo be successful in this role, you'll need:
\nProfessional registration with the NMC, HCPC or GPhC
\nIndependent Prescriber qualification
\nAdvanced assessment and minor illness qualification (Level 6 or 7)
\nProven experience of autonomous clinical practice
\nStrong clinical decision-making skills and ability to manage complex presentations
\nExcellent communication and interpersonal skills
\nCommitment to continuous professional development and reflective practice
\nEvidence of service improvement and innovation

\n2 Positions hours per week
