Role Overview

The Physical Therapist (PT) is responsible for assessing residents, creating tailored treatment plans, and delivering specialized therapy services aimed at enhancing strength, mobility, balance, safety, and overall functional independence. The PT collaborates closely with physicians, nursing staff, therapy team members, residents, and their families to optimize rehabilitation results and improve quality of life.

Key Responsibilities
  • Conduct thorough physical therapy evaluations.
  • Create personalized treatment plans based on individual resident needs, objectives, and physician directives.
  • Deliver skilled therapeutic interventions to enhance mobility, strength, balance, endurance, and functional independence.
  • Evaluate and recommend assistive devices, mobility aids, and safety modifications.
  • Track resident progress and modify treatment plans as necessary.
  • Educate residents, families, and caregivers about treatment plans and discharge recommendations.
  • Maintain precise and timely documentation in line with regulatory and professional standards.
  • Engage in interdisciplinary care planning meetings.
  • Ensure adherence to all federal, state, facility, and professional practice regulations.
Qualifications
  • Current Colorado Physical Therapist (PT) license in good standing.
  • Preferred experience in Skilled Nursing Facilities, rehabilitation, or long-term care.
  • Strong skills in clinical assessment and treatment planning.
  • Excellent communication and interpersonal skills.
  • Ability to work autonomously while effectively collaborating with an interdisciplinary team.
  • Dedication to providing outstanding resident-centered care.
Preferred Experience
  • Experience in Skilled Nursing Facilities (SNF).
  • Background in orthopedic rehabilitation, neurological conditions, balance training, fall prevention, and post-surgical recovery.
  • Familiarity with Medicare Part A, Part B, and managed care rehabilitation services.
