Trinity Health is seeking a compassionate and skilled Advanced Practice Provider (APP) to join our Family Medicine team. The ideal candidate will play a vital role in providing high-quality care to patients of all ages, promoting wellness, and managing both acute and chronic health conditions. As an integral part of our family medicine practice, the APP will work alongside physicians and other healthcare professionals to ensure comprehensive care for our diverse patient population.
\nKey Responsibilities:

- Provide primary and preventive care to patients across the lifespan, including children, adults, and elderly. \n
- Conduct comprehensive physical exams, obtain medical histories, and perform diagnostic tests. \n
- Diagnose, treat, and manage common acute and chronic medical conditions. \n
- Prescribe medications, order treatments, and provide patient education on disease prevention, treatment plans, and lifestyle modifications. \n
- Manage patient follow-up care and ensure continuity of care across various settings (inpatient, outpatient, etc.). \n
Qualifications:

- \n \n Licenses and Certifications Required\n\n
- \n
- \n \n Current ND Board NP/PA License required.\n\n
- \n \n Cardio Pulmonary Resuscitation (CPR) training, with certification every two (2) years, preferred.\n\n
- \n \n ACLS certification, with certification every two (2) years, preferred.\n\n

- \n \n Educational Requirements\n\n
- \n
- \n \n Education as required to obtain and keep current ND NP/PA license.\n\n

- \n \n Experience Requirements\n\n
- \n
- \n \n Extensive knowledge of established professional medical concepts, principles and practices, required.\n\n
- \n \n Experience in specialized medical areas, preferred.\n\n
- \n \n CPT coding knowledge, preferred.\n\n
