Position Overview and Qualifications

This role involves delivering direct patient care in alignment with physician directives while adhering to established standards of practice and organizational policies. You will be responsible for implementing care plans, administering treatments, and collaborating with team members to achieve favorable patient outcomes.

Your responsibilities will include:

  • Adhering to the care plan, administering treatments, and collaborating with team members to ensure positive patient outcomes.
  • Providing direct patient care as outlined in the State Nurse Practice Act, including specialized responsibilities related to end-of-life care.
  • Coordinating hospice services and effectively communicating identified needs to the interdisciplinary team, which includes Hospice Aides, the Director of Clinical Services – Hospice, and Chaplains.
  • Delivering appropriate pain and symptom management.

Qualifications required for this position:

  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Registered Nurse (RN) license is required.
  • A Bachelor's Degree must be completed within 90 days of employment commencement.
  • At least one year of relevant experience is required.
