Helen Porter Rehabilitation & Nursing, part of University of Vermont Health and Porter Medical Center, is seeking a compassionate and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our Memory Care Unit on the evening shift.

What You'll Do

\n

As an RN or LPN on our Memory Care Unit, you will:

\n
    \n
  • Provide skilled nursing care to residents with Alzheimer's disease and other memory-related conditions
  • \n
  • Assess, monitor, and document resident health status
  • \n
  • Administer medications and treatments according to physician orders
  • \n
  • Respond appropriately to changes in resident condition
  • \n
  • Collaborate with nursing assistants, providers, therapists, and interdisciplinary team members
  • \n
  • Promote resident safety, comfort, and emotional well-being
  • \n
  • Support residents with dignity, patience, and compassion
  • \n
  • Maintain accurate clinical documentation and communication
  • \n
  • Help create a positive, structured environment that supports residents' unique needs

Qualifications

RN Candidates

  • Current Vermont Registered Nurse (RN) license or eligibility for Vermont licensure
  • \n

LPN Candidates

  • Current Vermont Licensed Practical Nurse (LPN) license or eligibility for Vermont licensure
  • \n

Preferred Qualifications

  • Experience in memory care, dementia care, long-term care, skilled nursing, rehabilitation, assisted living, hospital, or acute care settings

  • Strong clinical assessment and critical thinking skills
