  • Schedule Full Time: Shared hours: Monday Friday 8am 5pm

Duties & Responsibilities

\n

A typical day may include:

\n
    \n
  • Provide comprehensive outpatient primary care patients 65 and older.
  • \n
  • Managing chronic conditions and developing personalized treatment plans
  • \n
  • Performing preventative care, wellness visits, and routine screenings
  • \n
  • Collaborating with nurse practitioners, specialists, and care teams to optimize patient outcomes
  • \n
  • Maintaining accurate and timely electronic health records documentation
  • \n
  • Participating in quality improvement initiatives to enhance patient care and clinic operations
  • \n
  • Staying up to date on evidence-based medical practices and advancements in primary care
  • \n
  • Engaging in community outreach and health education efforts when applicable
  • \n

Requirements

\n

  • Board Certified or Eligible in Family Medicine, Internal Medicine, or Med-Peds
  • \n
  • At least 5 years of post-residency clinical experience
  • \n
  • Strong clinical and diagnostic skills
  • \n
  • Commitment to patient-centered care and building lasting relationships
  • \n
  • Good communication and team skills
  • \n
  • Interest in working in a community-focused practice
  • \n
  • Willingness to work some early mornings, evenings, and rotating weekends
  • \n
  • Outpatient primary care experience (preferred)
  • \n
