Duties & Responsibilities
\nA typical day may include:
\n- \n
- Provide comprehensive outpatient primary care patients 65 and older. \n
- Managing chronic conditions and developing personalized treatment plans \n
- Performing preventative care, wellness visits, and routine screenings \n
- Collaborating with nurse practitioners, specialists, and care teams to optimize patient outcomes \n
- Maintaining accurate and timely electronic health records documentation \n
- Participating in quality improvement initiatives to enhance patient care and clinic operations \n
- Staying up to date on evidence-based medical practices and advancements in primary care \n
- Engaging in community outreach and health education efforts when applicable \n

Requirements
\n- \n
- Board Certified or Eligible in Family Medicine, Internal Medicine, or Med-Peds \n
- At least 5 years of post-residency clinical experience \n
- Strong clinical and diagnostic skills \n
- Commitment to patient-centered care and building lasting relationships \n
- Good communication and team skills \n
- Interest in working in a community-focused practice \n
- Willingness to work some early mornings, evenings, and rotating weekends \n
- Outpatient primary care experience (preferred) \n
