\nFairview is seeking a Nurse Practitioner to join our primary care practice at M Health Fairview Clinic River Falls. Join a collaborative and growing practice with a supportive, experienced care team dedicated to providing high-quality, patient-centered care.

- Established primary care practice with a collaborative team-based environment \n
- Patient encounters average 18 patients per day \n
- 1.0 FTE = 34 patient contact hours per week \n
- Openings: 0.8 to 1.0 FTE (Full Time) flexible scheduling options \n
- Fully integrated Electronic Medical Record (EPIC) \n
Responsibilities

- Assess health status for specific patient populations using advanced practice knowledge and skills. \n
- Obtains relevant health and medical history. \n
- Performs a physical examination based on age and medical history. \n
- Performs/orders preventative and diagnostic procedures based on the patient's age and history. \n
- Identifies health and medical risk factors. \n
- Identifies acute and chronic health problems. \n
- Appropriately orders and accurately interprets diagnostic results. \n
- Develops and implements an individualized plan of care. \n
- Establishes practice agreements with collaborating physician. \n
- Collaborates with health care team members in developing realistic care expectations. \n
- Determines effectiveness of treatment plan with documentation of patient care outcomes. \n
- Revises and modifies the care plan as needed to achieve health and medical goals. \n
- Refers care to other health care professionals and community agencies as needed. \n
- Prescribes pharmacologic (including controlled substances) and non-pharmacologic agents. \n
- Communicates effectively with patients and family for understanding of diagnosis, treatment plan, and need for follow-up care. \n
- Provides education to patients, families or caregivers as needed. \n
- Documents according to established guidelines to facilitate ongoing quality care in a timely manner. \n
- Responds in a timely manner to phone calls regarding patient needs. \n
- Contributes to effective and efficient practice management policies and practices that support patient access to medical care. \n
- Participates in access planning for applicable area or population. \n
- Supports appropriate billing for professional service through timely documentation and maintaining adequate knowledge of FVC billing policies and procedures. \n
- Adheres to all relevant policies, procedures, and practice guidelines. \n
- Assists with the preparation for JCAHO and other regulatory surveys. \n
- Manages patient care through appropriate use of available resources. \n
- Appropriately prescribes and orders ancillary services. \n
- Utilizes Fairview referral network appropriately. \n
- Manages chronic health/disease conditions. \n
- Provides leadership for clinical and support staff. \n
- Serves as a resource to support staff, enhancing quality of care. \n
- Participates in the training, orientation, and evaluation of clinical and support staff. \n
- Participates in appropriate clinical investigation relevant to specific patient population. \n
- Responds to nursing staff educational needs in formal and informal venue demonstrating nursing procedures, charting procedures, and reviewing pathophysiology of disease and new treatment modalities. \n
- Serves as consultant and role model for nursing staff. \n
- Maintains competence and professional development. Engages in continuing education. \n
- Is active in professional and other appropriate organizations. \n
- Demonstrates behaviors that support performance improvement activities. \n
- Demonstrates ability to provide care or service adjusting approaches to reflect developmental level and cultural differences of population served. \n
Required Qualifications
- \n
- Graduate of an accredited school of nursing and formal nurse practitioner training program. \n
- BC/BE through the AANP or ANCC \n
- Licensed or ability to obtain APRN license with the Minnesota Board of Nursing \n
- Ability to obtain and /or maintain DEA certification for the State of Minnesota \n
- \n
Licensed or ability to obtain Wisconsin RN licensure and Advanced Practice Nurse Prescriber (APNP) certification through the Wisconsin Board of Nursing.

Ability to obtain and/or maintain DEA certification for the State of Wisconsin.
\n - Meet threshold criteria/qualifications for Credentialing and Privileges \n
$133,156 - $147,952 Annual *based on full time (1.0 FTE)
