- MD or DO degree.
- Board Eligible or Board Certified.
- Possession of an active and unrestricted medical license.

Education and Experience- Doctorate of Medicine is required.
- Preferred: Over 5 years of experience in Family Practice.
- Preferred: Over 5 years of experience in Gynecology.
- Basic Life Support (BLS) certification is preferred.
- Active physician license is required.
- Drug Enforcement Administration (DEA) license is required.
