Nazareth Family Medicine, part of Trinity Health Mid-Atlantic, is seeking a Board-Certified/Board-Eligible Family Medicine Physician to support continued primary care expansion and access initiatives across the Philadelphia market.

This opportunity offers a 100% outpatient setting, strong clinical support, and the ability to build a patient panel within a mission-driven, community-focused health system.

Qualifications

  • MD or DO from an accredited institution

  • Board Certified or Board Eligible in Family Medicine

Role Responsibilities

  • Provide comprehensive primary care for patients across the lifespan
  • \n
  • Diagnose and manage acute and chronic conditions
  • \n
  • Emphasize preventive care and population health management
  • \n
  • Collaborate with interdisciplinary teams to optimize patient outcomes

  • Maintain timely and accurate documentation
  • \n
  • Participate in quality and performance improvement initiatives

  • Full-time outpatient schedule (34 patient-facing hours)
