Our organization is currently looking for a full-time, Board-Certified or Board-Eligible physician specializing in Family Practice or Internal Medicine to join our outpatient clinic located in Fairview, Oregon. This role will be designated as the Primary Care Clinical Practice Manager Physician. As part of the Portland VA Medical Center, you will deliver services within a comprehensive network of care facilities and outpatient clinics throughout Oregon.
The primary care provider will operate as a highly skilled, independent practitioner, managing patient care while collaborating with an interdisciplinary team. You will have access to cutting-edge medical technologies and state-of-the-art healthcare practices.
Key Responsibilities:- Lead and manage a panel of approximately 900 patients as part of the Patient Aligned Care Team (PACT) model.
- Provide comprehensive care for Veterans, addressing both acute and chronic conditions such as diabetes, hypertension, chronic obstructive pulmonary disease, and more.
- Work collaboratively with a team that includes a Primary Care Provider (PCP), Registered Nurse (RN), Licensed Practical Nurse (LPN), and Medical Support Assistant.
- Promote staff education and development initiatives.
- Commit to professional growth and foster positive labor relations.
- Must be a U.S. Citizen.
- Possess a current, full, and unrestricted medical or surgical license in the U.S.
- Board Certification in Family Practice or Internal Medicine is required.
This is a full-time position, operating Monday through Friday, with minimal on-call responsibilities for abnormal lab results. There are no weekend or holiday shifts.
