\nWe are seeking a PRN Physical Therapist (PT) to join our dedicated rehabilitation team. This position offers flexibility and the opportunity to make a meaningful impact on residents' lives in a rewarding setting.\n

Position Summary: \n
\nAs a PRN Physical Therapist, you will evaluate and treat residents to improve their mobility, strength, and functional independence. You will work collaboratively with a multidisciplinary team to deliver high-quality, individualized care.\n
\n
Key Responsibilities: \n
\n

\n
• Perform comprehensive evaluations of residents' physical abilities and needs.
\n
• Develop and implement personalized treatment plans to improve strength, mobility, balance, and overall function.
\n
• Provide therapeutic interventions, including exercises, manual therapy, and training on assistive devices.
\n
• Monitor residents' progress and modify therapy plans as needed to achieve goals.
\n
• Collaborate with an interdisciplinary team to ensure holistic care.
\n
• Educate residents and their families on therapy goals, exercises, and strategies for continued improvement.
\n
• Accurately document evaluations, treatment sessions, progress notes, and discharge plans in compliance with regulatory and facility standards.
\n
• Participate in quality improvement initiatives and contribute to the rehabilitation department's success.
\n

Qualifications: \n
\n

\n
• Doctorate or Bachelor's/Master's degree in Physical Therapy from an accredited program.
\n
• Current and valid Physical Therapist license in the state of Colorado.
\n
• Experience in skilled nursing or rehabilitation settings is preferred.
\n
• Strong communication and teamwork skills with a resident-centered approach.
\n
• Ability to work independently and adapt to a flexible PRN schedule.
\n
