\n JOB SUMMARY/PURPOSE \n -
\n Provides clinical pharmacy services to ambulatory patients. Provides comprehensive medication management in multiple areas of practice, including but not limited to anticoagulation, perioperative management of anticoagulants and antiplatelets, anemia management with erythropoiesis stimulating agents, diabetes, hypertension, hyperlipidemia, heart failure, COPD, asthma and opioid use disorder. Ensures departmental efficiencies and compliance with regulatory bodies. Instructs other pharmacists, pharmacy residents and pharmacy students. \n -
\n DEPARTMENT DESCRIPTION \n - The Medication Management Clinic provides comprehensive pharmaceutical care and care coordination to ambulatory patients who are on high-risk medications or who require disease state management. \n

EXPERIENCE/EDUCATION/QUALIFICATIONS\n
- \n
- Current unencumbered Oregon Pharmacist and Pharmacist Preceptor licensure required. \n
- Healthcare Provider Level BLS required within 90 days of hire. \n
- Previous ambulatory care clinic experience preferred. Alternatives may include completion of a PGY 1 residency with rotations in ambulatory care and anticoagulation rotation, completion of a PGY 2 residency in ambulatory care, or board certification in ambulatory care. \n
