    Preventive care (immunizations, screenings)

    Chronic disease management (diabetes, hypertension, COPD, heart disease)

  • \n

    Acute care visits

    • \n
  • \n

    Mental health screenings (PTSD, depression, substance use)

    • \n
  • \n

    Geriatric care and care coordination

    • \n

Support Staff:

  • \n

    Physicians & mid-levels

    • \n
  • \n

    RN staff

    • \n
  • Social workers
    • \n
  • \n

    On-site specialty providers

Requirements:

    \n
  • \n

    Board-Certified or Board Eligible in Family Medicine or Internal Medicine

    • \n
  • \n

    DEA license (schedules 2–5, fee paid)

    • \n
  • \n

    BLS & ACLS (American Heart Association only)

    • \n
  • \n

    EMR: CPRS (training provided if needed)

    • \n
  • \n

    Any active state license accepted!

    • \n
