Hospice & Palliative Medicine, Geriatric Physician - Expanding Transitional Care Program
Job Description
Augusta Medical Group is seeking a compassionate, relationship-driven physician to join a growing community-based geriatric and transitional care program serving the Shenandoah Valley. This is a growth-focused role with a strong opportunity to help expand services, shape workflows, and build the future of geriatric care across the region.
\nThe position provides care across 7 assisted living facilities and a hospital-based SNF/rehab unit, supporting approximately 234 assisted living patients and ~12 SNF patients (average daily census). The physician will work within a collaborative, supportive team that includes an APP, CMA, administrative support, and strong partnership with local psychiatry services to support a complex behavioral health population.
\nKey responsibilities include:

- Providing comprehensive primary care to geriatric patients in assisted living and SNF settings \n
- Managing chronic disease, acute care needs, and preventive care \n
- Conducting facility-based visits across 12 sites per day less than 20 minutes commuting distance from main location. \n
- Coordinating care across facilities, hospitals, and specialty providers \n
- Building strong relationships with patients, families, and facility staff \n
- Supporting and helping grow the program, including workflow development and quality initiatives \n
Practice highlights:

- ~10 patients per day in a community-based setting \n
- Shared home-based call across Transitional Care, Hospice, and Palliative teams \n
- Weeknights: 6 PM 6 AM (rotating) \n
- Weekends: 1:9 rotation \n
- Collaborative, team-oriented environment with strong system support \n
- Interactions with residents given vibrant Internal Medicine Residency \n

Ideal candidate:

- Board-certified/board-eligible in Family Medicine, Internal Medicine, or Geriatric Medicine \n
- Passion for geriatric and underserved populations \n
- Strong communicator and team player \n
- Interested in helping grow and develop a rapidly expanding service line \n
- Experience in assisted living, SNF, or home-based care preferred \n
Salary
\nStarting Salary $335,240
\nSign-On- $30,000
\nRelocation: $10,000 (receipt based)
\nCME: $4200
