As a Nurse Practitioner (NP) or Physician Assistant (PA) within our Senior Primary Care division, you will collaborate with a diverse team that includes physicians, pharmacists, care coaches, and behavioral health specialists, all committed to delivering tailored, high-quality care.

This position is based at our Med Center and Boerne clinics in the San Antonio area, reporting directly to the Associate Medical Director.

Your Responsibilities

  • Deliver comprehensive care to seniors in accordance with clinical standards.
  • Engage daily with your care team to ensure seamless, high-quality service.
  • Oversee referrals, coordinate with hospitals/SNF, and manage DME and home health services.
  • Accurately document care with assistance from quality-focused coders.
  • Participate in on-call rotations and contribute to growth initiatives at your center.
  • Demonstrate a commitment to providing value-based care.

Qualifications We Seek

Essential:

  • A Master’s Degree in Nursing or completion of a PA program with board certification.
  • An active, unrestricted NP or PA license in the state where you will practice.
  • Successful candidates will undergo TB screening if selected for the role.

Preferred:

  • A minimum of 2 years of experience in value-based care, primary care, or Medicare Advantage environments.
  • An active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.

Compensation Range

The salary range provided reflects a good faith estimate of the starting base pay for full-time employment (40 hours per week) at the time of posting. Actual compensation may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.

$123,800 - $170,300 per year

This position is eligible for a bonus incentive plan based on individual and/or company performance.
