Position Overview:

Pediatric Nurse Practitioner (PNP) or Physician Assistant (PA) needed for an FQHC in Hornell, NY

Schedule:

• Monday: 8:00 AM - 5:30 PM
\n
• Tuesday - Thursday: 8:00 AM - 8:00 PM
\n
• Friday: 8:00 AM - 5:00 PM
\n

Flexible schedule with 36 hour work week

You will join a collaborative team, working with two other Advanced Practice Providers (APPs), and partner with MD/DO in delivering comprehensive, patient-centered care.

Responsibilities:

\n
• Conduct History of Present Illness (HPI) assessments and physical examinations.
\n
• Diagnose and develop individualized treatment plans for patients.
\n
• Prescribe medications per New York State statutes and professional guidelines.
\n
• Perform minor surgical procedures.
\n
• Provide counseling and education to patients regarding their treatment plans.
\n
• Generate referrals for specialized care as needed.
\n
• Contribute to medical decision-making and support the Care Team.
\n
