\nWe are seeking an APP who is looking for something different than the traditional hospital or corporate healthcare environment. This role offers a high degree of clinical autonomy, flexibility, and independence. As a Wound Care provider with HWMS, you will manage your own patient panel, structure your daily schedule, and provide care in a way that prioritizes outcomes. This is an ideal role for a provider who is confident in their clinical decision-making, values patient relationships, and prefers a more flexible and self-directed work environment.\n

\nCare Coordination\n
\n
\n* Collaborate with home health agencies, skilled nursing facilities, and referring providers\n
\n* Communicate treatment plans and updates to the broader care team\n
\n* Support transitions of care following hospital or SNF discharge\n
\n* Educate patients and caregivers on wound management and prevention\n
\n
\nDocumentation & Compliance\n
\n
\n* Maintain accurate, timely, and compliant documentation\n
\n* Ensure documentation meets Medicare and payer requirements\n
\n* Follow HWMS clinical protocols and workflows\n
\n* Participate in periodic chart reviews and quality initiatives\n
\n
\nQualifications Required\n
\n
\n* Active, unrestricted North Carolina license\n
\n* Board Certification (FNP, AGNP, PA-C, or similar)\n
\n* DEA license (or ability to obtain)\n
\n* Valid driver's license and reliable transportation\n
\n
\nPreferred\n
\n
\n* Wound care experience or certification (CWOCN, CWON, WCC, or similar)\n
\n* Experience in home health, SNF, or post-acute care settings\n
\n* Familiarity with Medicare documentation and billing practices\n
\n
