Position Overview and QualificationsThe RN Case Manager plays a crucial role in fostering patient-centered care by overseeing the care plan during the patient's stay. This includes managing the duration of the stay, ensuring effective resource management, and collaborating with a multidisciplinary team to create a safe and suitable discharge plan.
Key Responsibilities:- Deliver case management services for both inpatient and observation patients as assigned.
- Identify patients at risk for negative outcomes during transitions between different levels of care.
- Conduct thorough assessments of patients' psychosocial, medical, and discharge needs, along with evaluating available resources for patients and families.
- Reassess patients' clinical conditions as necessary, considering readmission risks and developing strategies to prevent them, including education on healthcare resources and community support.
- Coordinate care plans and drive discharge processes by collaborating with the healthcare team, especially the patient's physician, to ensure smooth transitions.
- Work with Social Services to confirm that post-acute medical needs and care levels are appropriate.
- Take responsibility for timely referrals to Social Services when psychosocial health risk factors are identified.
- Engage patients and their families in identifying needs and expectations to set mutual, realistic goals.
- Evaluate care progression using evidence-based tools and approved criteria (InterQual) throughout the care episode, escalating issues as necessary.
- Make appropriate referrals to third-party payers and disease management programs for patients with chronic conditions or recurring needs.
- Facilitate patient throughput with a focus on effective transitions, quality, and efficiency.
- Document professional recommendations, discharge plans, care coordination efforts, and case management activities to ensure clear communication among the healthcare team.
- Align patient needs with available resources to guarantee safe discharges and transitions.
- Adhere to the organization’s “Code of Conduct” and “Mission and Value Statement.”
- Current RN License (mandatory)
- Associate Degree in Nursing or Nursing Diploma (mandatory)
- Bachelor’s Degree in Nursing (preferred)
- Minimum of 2 years of experience in case management or 3 years in clinical nursing (mandatory)
- Experience with InterQual (preferred)
