Job Overview and Qualifications

In this role, you will be responsible for delivering direct patient care in alignment with physician directives, adhering to established standards of practice, and following company policies and procedures. You will implement the care plan, administer treatments, and collaborate with team members to achieve positive outcomes for patients.

Your responsibilities will include:

  • Adhering to the care plan, administering treatments, and collaborating with team members to ensure positive patient outcomes.
  • Providing direct patient care as outlined in the State Nurse Practice Act, including specialized tasks related to end-of-life care.
  • Participating in the coordination of hospice services and effectively communicating identified needs to the interdisciplinary team, including Hospice Aides, the Director of Clinical Services – Hospice, and Chaplains.
  • Delivering appropriate pain and symptom management.
Qualifications Required:
  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • Registered Nurse (RN) license.
  • Valid Driver's License (DL).
  • Associate Degree in Nursing.
