Job Summary and Responsibilities
As ourGeriatric Nurse Practitioner, you will helpolder adults, particularly those with Alzheimer's and Dementia,receive specialized, comprehensive primary care and off-site home visitsso they canimprove their health outcomes, manage complex geriatric conditions, and maintain a higher quality of life.
\n\nIn this role, you willfocus on Alzheimer's and Dementia care, assuming clinical responsibility for patient histories, physical examinations, accurate diagnoses, and care management.You will be expected tomanage a project volume of 250 patients, conduct new consults and follow-up visits in clinic and through home visits, and collaborate within a multidisciplinary team to ensure holistic patient care.
\n\nTo be successful in this role, you mustbe a licensed Nurse Practitioner with expertise in geriatrics and dementia care, demonstrate exceptional proficiency in clinical assessment and management, and possess strong communication and collaborative skills for diverse patient and team interactions.

- This position will be Alzheimers and Dementia Care focus \n
- Patients are seen in the office with some off-site home visits in the surrounding area \n
- Typical work week (hours worked; hospital vs. clinic) - option to work 4 or 5 days per week \n
- Working in clinic setting, as well as, off site visits throughout the community to accommodate the patients needs \n
- Typical patient visit load of 2 new consults a day and follow up visit \n
