Job Overview and Qualifications

Schedule: Monday to Friday, 8:00 AM - 4:30 PM

In the role of Case Manager, you will assist patients and their families throughout their healthcare journey. Your responsibilities will involve coordinating services, connecting resources, and crafting care plans tailored to each patient's specific needs. By collaborating with physicians, nurses, and department leaders, you will help facilitate safe transitions, ensure clear communication, and maintain consistent quality across the care continuum.

Your key responsibilities will include:

  • Assisting patients and families during program orientation by explaining rehabilitation philosophies, Medicare and insurance benefits, discharge criteria, and patient rights.
  • Coordinating educational sessions for patients, families, and caregivers to promote active participation in treatment planning, goal-setting discussions, and family conferences.
  • Creating and executing personalized treatment plans that align with the patient’s strengths, needs, and recovery objectives.
  • Conducting psychosocial assessments and discharge planning evaluations, thoroughly documenting findings and communicating needs within the care team.
  • Working closely with the Rehab Program Director, Facility Case Management Director, and interdisciplinary team to ensure seamless, patient-focused care.
  • Upholding the organization’s values of compassion, respect, and excellence in every interaction with patients and families.
Qualifications Required:
    • Registered Nurse (RN) license
    • Diploma or Associate Degree in Nursing
    • Preferred experience in a hospital setting
