Nurse Practitioner - Trinity Health PACE Fayetteville, NC

Become a part of our team, where you will play a vital role in delivering primary medical care alongside our respected PACE Organization (PO) Physicians. As a full-time Nurse Practitioner, you will be responsible for conducting evaluations, creating care plans, and ensuring that participants receive thorough care in a nurturing and supportive atmosphere.

Key Responsibilities:

• Work collaboratively with PO physicians to perform assessments and execute care plans.
• Engage in discharge planning and provide treatment for acute illnesses.
• Conduct home visits as required.
• Assist in the development of clinical policies and procedures.
• Participate in the provider on-call rotation.

Essential Qualifications:

• Master's degree in Nursing.
• 5-7 years of experience, ideally in long-term care settings.
• Valid nursing license to practice in North Carolina.
• CDS Certification, DEA Registration, and Board Certification are mandatory.
• Current BLS certification.
• Willingness to travel within the designated geographic area.
