Work Environment and Patient Demographics

Our facilities function as Skilled Nursing Facilities (SNFs), offering:

The mix of patients may vary slightly by facility. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life in a supportive, patient-focused environment.

Position Summary

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Qualifications and Experience
  • Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT; those graduating in 2003 or later must possess an MSPT or DPT)
  • Current registration/licensure in the applicable state, with an active license in good standing throughout employment
  • Preferred one (1) year of experience; however, new graduates are welcome to apply
  • CPR certification required upon hire or to be obtained during orientation, with ongoing maintenance of certification during employment
Job Responsibilities
  • Engage in community health initiatives and projects as appropriate
  • Proficiently use Microsoft Word, Excel, and email
  • Coordinate with patients, families, and support departments to effectively address patient needs
  • Exhibit proper body mechanics at all times
  • Make independent decisions when necessary
  • Understand and comply with practices, procedures, and regulations governing post-acute care facilities
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain professional relationships with colleagues, vendors, and others
  • Safeguard the confidentiality of all proprietary and confidential information
  • Adhere to company policies, including those related to harassment and compliance
  • Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
Essential Functions
  • Implement physical therapy treatment plans for patients
  • Set, evaluate, and adjust realistic, measurable, and functional goals
  • Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students
  • Document care accurately and in a timely manner
  • Utilize therapy software effectively and accurately
  • Provide excellent customer service and maintain a positive attitude towards patients
  • Assist in the safe evacuation of patients when necessary
  • Demonstrate reliable and consistent attendance
  • Employ reasoning skills and sound judgment in various situations
  • Collaborate effectively within an interdisciplinary team
  • Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently throughout the workday
  • Read, write, speak, and comprehend the English language
