Position Overview
The Care Manager Assistant - LPN plays a vital role in coordinating patient care and services. This position emphasizes quality care through a collaborative approach that involves monitoring, evaluating, and aligning services with the needs of patients, while adhering to all relevant laws, regulations, and organizational standards.
Education, Experience, and Licensure Requirements- Completion of a nursing diploma (either an associate's or bachelor's degree in nursing)
- Active and valid nursing license (LPN) in the relevant state, which must be maintained in good standing throughout employment
- Preferred: At least one (1) year of clinical experience in a post-acute care environment
- Preferred: Experience in case management, utilization review, and discharge planning
- Ability to produce written communications that are clear, concise, and well-structured
- Strong organizational skills with the capability to prioritize tasks and manage time effectively
- Contribute to the organization's objectives and support strategic initiatives
- Possess expert knowledge in the relevant field of practice
- Ability to make independent decisions when necessary
- Familiarity with the practices, procedures, and regulations governing operations in a post-acute care facility
- Implement and interpret departmental programs, goals, policies, and procedures
- Demonstrate proficiency in all competency areas, including general management duties, patient rights, and safety protocols
- Maintain professional relationships with colleagues, vendors, and other stakeholders
- Ensure confidentiality of all proprietary and sensitive information
- Adhere to organizational policies, including those related to harassment and compliance
- Exhibit integrity and professionalism by following the organization's Code of Conduct and completing required compliance training
- Effectively utilize relevant computer systems for data entry and retrieval
- Act as a liaison between external case managers, families, physicians, and community resources
- Provide training and education to patients, families, colleagues, and other care providers
- Implement standards of practice for care management, ethical performance, and care coordination
- Deliver excellent customer service and maintain a positive attitude towards patients
- Assist in the safe evacuation of patients when necessary
- Demonstrate reliable and consistent attendance
- Utilize reasoning skills and sound judgment in various situations
- Collaborate effectively within an interdisciplinary team
- Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving throughout the workday
- Read, write, speak, and comprehend the English language
