- MD or DO degree.
- Board Eligible or Board Certified.
- Possession of an active and unrestricted medical license.

Experience and Education- Preferred: Over 5 years of experience in Family Practice.

- Basic Life Support (BLS) certification preferred.
- Active medical license required.
