\nJOB SUMMARY
\n- \n
- The Level II Residential LPN is responsible for the assessment and management of withdrawal signs/symptoms of clients presenting to the unit. \n
- These include initial medical assessments of clients during intake, and providing appropriate nursing care form Medical Detox, Medically Monitored Detox and Residential Services. \n
- The schedule for this position is 7pm-7am. \n
JOB DUTIES/RESPONSIBILITIES

\n1. Review of admission and discharge criteria before admission t the unit and refer to
\nCoordinator/Medical Director as appropriate.
\nPerform the initial medical assessment of individuals within 2 hours of admission.
\nMakes a determination of the appropriate detox protocol upon admission.
\n2. Complete all documentation in compliance with CARF and program requirements.
\nMaintains accurate records of nursing interventions.
\nMonitoring of treatment plan effectiveness and compliance, including obtaining consents.
\nReview with client ASAM placement criteria and obtain signature Q shift.
\nPerform TB testing on each client upon admission and document results 48-72 hours later.
\n3. Monitor appointment calendar and available beds each shift/day to maintain full status.
\nUpon admission, determine client's discharge date/time and place on board.
\n4. Maintains accurate records of nursing interventions.
\nAdminister prescribed, routine and PRN medications as scheduled no more than 1 hour ac or 1 hour pc of prescribed time.
\nPerform charting 3 X per 8 hour shift, 4 X per 12 hour shift and PRN .
\nComplete nursing assessment within 2 hours.
\nMonitor Clients' VS/ withdrawal signs/symptoms per CIWA/COWS upon admission, shift X 3 days then QD if score is >10.
\nMonitor significant changes in the clients' condition Q shift and make appropriate referral.
\nEnsures that physicians' orders are evaluated by nursing staff within 2 hours of the order.
\nCompletes nursing assessments within time allotted, III.7D/MMCD and RR5 ? 2 hours.
\nMakes client rounds and notes problems for follow-up.
\nAssist clients in obtaining prescribed medications.
\nMaintains harmonious, productive interpersonal relationship with clients, co-workers, visitors and families.
\nAbility to complete all work assignments within shift assigned.
\n5. Monitoring of treatment plan effectiveness and compliance, including obtaining consents.
\nObtain Medication Consents on all medications upon admission/PRN prior to medicating client.
\nCompletes the required TX plan with client upon admission.
\nMonitor electronic record for completed documentation.
\n6. Functions as a member of a multi-disciplinary management team.
\nAttends shift reports each shift with IC/OG nursing staff without tardiness.
\nParticipates in call rotation as scheduled, returns calls within 15 min.
\nAttends all mandatory staff meetings monthly.
\nMust be able to perform shift work as needed to include overtime, weekends and holidays.
\nMust be punctual, maintain a good attendance record.
\nAble to maintain a level of supervising and capable of following and interpreting policy and procedures as they relate to the clients and staff in the detox unit.
\nAbility to relate positively and be emotionally supportive to individuals who are chemically dependent/co-dependent.
\nAble to establish professional client boundaries, effectively sets limits with clients.
\nObserve clients for changes in physical/mental conditions and documents changes for consultation with the Medical Director/Medical Services coordinator.
\nNursing Staff will keep other staff members informed on status of the clients.
\nPerforms other duties as assigned by the Medical Service coordinator.
QUALIFICATIONS - Medically Monitored Crisis Detox LPN
\nEducation / Knowledge:
\n- \n
- Graduate of an accredited Licensed Practical Nurse Program and possess a current nursing license. \n
- Possess a working knowledge of basic nursing techniques. \n
- Demonstrates ethical and appropriate behavior for a clinical environment to include, but not limited to, dignity, patient's rights and privacy, and the maintenance of confidentiality of patient information. \n
- The ability to retrieve, communicate and present data and information both verbally and in writing required. \n
- LPN with current Tennessee license and experience in psychiatric nursing preferred. \n
- LPN certification as Psychiatric/Mental Health Nurse preferred. \n
- Should have sound knowledge of psychiatric medications, diagnoses, treatment modalities and interventions. \n
- Ability to use computer. \n
- Must be CPR certified or willing to become certified. \n
- Will also need to complete the required Handle With Care Training. \n
- Assist and evaluates the need for emergency transfer to hospital in cases of serious injury, illness. \n
- Knowledge of substance abuse, chemical dependency, co-occurring disorders and recovery. \n
- Ability to reason and make judgments regarding client's care. \n
- Must be flexible with work scheduling. \n
- Participates in On-Call rotation as scheduled. \n
- Class room training in Pharmacology, medical terminology, charting and nursing process. \n
Experience:
\n- \n
- Prefer 1 years' experience in substance abuse and treatment techniques. \n
- Experience in psychiatric nursing preferred. \n
- Experience in working with special populations including individuals with Substance Use/Abuse and/or Addiction, Severe and Persistent Mental Illness, Co-Occurring Disorders, and Dual Diagnosis Disorders preferred. \n
- Computer experience is helpful. \n
- Experience working in a crisis setting preferred. \n
Compensation:
\n Salary is based on relevant experience and education. Applicable shift differentials are paid for eligible evening and overnight shifts. More information to be provided during the interview process.
