\n Position Overview:
\n As a Primary Care Physician at the Port Charlotte CBOC within the Bay Pines VA Healthcare System, you will deliver longitudinal, whole-person care to an assigned patient panel using the VA’s Patient Aligned Care Team (PACT) model. You’ll focus on prevention, chronic disease management, and care coordination—supported by an interdisciplinary team.

\n Provide comprehensive primary care to an assigned panel of adult Veterans
\n Diagnose and manage acute and chronic medical conditions
\n Deliver preventive care, health maintenance, and wellness counseling
