We are currently looking for a PRN Physical Therapist (PT) to become a part of our committed rehabilitation team. This role provides flexibility and the chance to significantly impact the lives of our residents in a fulfilling atmosphere.

Position Overview:
In the role of PRN Physical Therapist, you will assess and treat residents to enhance their mobility, strength, and functional independence. You will collaborate with a multidisciplinary team to provide high-quality, personalized care.

Primary Responsibilities:

  • Conduct thorough evaluations of residents’ physical capabilities and requirements.
  • Create and execute tailored treatment plans aimed at improving strength, mobility, balance, and overall functionality.
  • Deliver therapeutic interventions, including exercises, manual therapy, and guidance on assistive devices.
  • Track residents’ progress and adjust therapy plans as necessary to meet established goals.
  • Work alongside an interdisciplinary team to ensure comprehensive care.
  • Inform residents and their families about therapy objectives, exercises, and strategies for ongoing improvement.
  • Maintain accurate documentation of evaluations, treatment sessions, progress notes, and discharge plans in accordance with regulatory and facility standards.
  • Engage in quality improvement initiatives and contribute to the success of the rehabilitation department.

Qualifications:

  • Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited institution.
  • Current and valid Physical Therapist license in Colorado.
  • Preferred experience in skilled nursing or rehabilitation environments.
  • Excellent communication and teamwork abilities with a focus on resident-centered care.
  • Capability to work independently and adapt to a flexible PRN schedule.
  • Commitment to delivering compassionate and effective therapy services.
