Position Overview:
In the role of PRN Physical Therapist, you will assess and treat residents to enhance their mobility, strength, and functional independence. You will collaborate with a multidisciplinary team to provide high-quality, personalized care.
Primary Responsibilities:
- Conduct thorough evaluations of residents’ physical capabilities and requirements.
- Create and execute tailored treatment plans aimed at improving strength, mobility, balance, and overall functionality.
- Deliver therapeutic interventions, including exercises, manual therapy, and guidance on assistive devices.
- Track residents’ progress and adjust therapy plans as necessary to meet established goals.
- Work alongside an interdisciplinary team to ensure comprehensive care.
- Inform residents and their families about therapy objectives, exercises, and strategies for ongoing improvement.
- Maintain accurate documentation of evaluations, treatment sessions, progress notes, and discharge plans in accordance with regulatory and facility standards.
- Engage in quality improvement initiatives and contribute to the success of the rehabilitation department.
Qualifications:
- Doctorate or Bachelor’s/Master’s degree in Physical Therapy from an accredited institution.
- Current and valid Physical Therapist license in Colorado.
- Preferred experience in skilled nursing or rehabilitation environments.
- Excellent communication and teamwork abilities with a focus on resident-centered care.
- Capability to work independently and adapt to a flexible PRN schedule.
- Commitment to delivering compassionate and effective therapy services.
