RN Case Manager (Home Health) Location: Lexington, KY (Onsite/Field-Based) Schedule: Monday–Friday, 8:00 AM–5:00 PM + on-call (1 weekend/month + 1 weekday/week) Pay: $56.14/hr Summary and Responsibilities Hiring an experienced RN Case Manager to support high-risk members in a home health setting. This RN Case Manager role focuses on comprehensive assessments, individualized care plans, and coordinated transitions of care.
