Assignment Dates: September 14th through October 29th

Schedule/Shifts: Monday - Thursday (Mon 8a-6p, Tues 8a-5p, Wed 8a-6p, Thur 7a-1p)
\n
Patient Volume: 8-10
\n
Patient Type: 95% Adult 5% Adolescent and Peds
\n
Procedures: Prophies, Perio Main, root planning, x-rays (Pan and BWX)
\n
Support Staff: 2 DAs, 2 Hygienists, 2 Drs
\n
Type of Charting: Eaglesoft
\n
Requirements: Active WI License, BLS
\n
Reason for Need: Medical Leave
\n
Credentialing Timeframe: 1-2 weeks
\n
Travel and Lodging Provided? Prefer Mileage only
