Job Opportunity: Skilled Nursing Facility Physician (SNFist)

Our organization, located in Spartanburg, SC, is actively looking for a full-time Board Certified/Board Eligible Internal Medicine or Family Medicine physician, or a fellowship-trained Geriatrician, to join the Medical Group of the Carolinas - Senior Health team. This role involves providing clinical care to inpatients at a skilled nursing facility.

Key Responsibilities:
  • Deliver direct medical care and emergency services to geriatric residents.
  • Collaborate with interdisciplinary care teams and act as a liaison with families regarding care decisions.
  • Organize and facilitate daily Care Team Meetings within the facility.
  • Monitor progress on care plans and identify early warning signs for potential readmissions during the critical post-discharge phase.
  • Engage in discussions about goals of care, transition planning, and advanced directives with patients and their families.
  • Serve as the attending physician for patients transitioning to Hospice care, providing support in symptom management at the end of life.
  • Supervise Nurse Practitioners delivering care in Skilled Nursing Facilities.
  • Participate in Quality Improvement, Peer Review, and Care Management Meetings as required.
  • Opportunities to teach medical students and primary care residents.
  • Engage in research focused on innovative care models.
Requirements and Qualifications:
  • Board Certified/Board Eligible in Internal Medicine or Family Medicine, or fellowship-trained in Geriatrics.
  • Strong commitment to providing high-quality care to geriatric patients.
Working Conditions:
  • Flexible scheduling options available.
  • On-call rotation of 1:3 weeks, including weekends (phone only), with Nurse Practitioners as the first point of contact.
