We are excited to announce an opening for a Primary Care Physician to become part of our multi-specialty group at our innovative Health Park in Mays Landing, southeastern New Jersey's largest health system and a recipient of the Malcolm Baldrige National Quality Award. This role offers a unique chance to provide exceptional, patient-focused care while contributing to the evolution of healthcare in our community.

Practice Highlights & Innovations

• Team-Based, Integrated Care:
  • RN Care Managers: Collaborate with all providers to manage risk-stratified patient populations and ensure coordinated care for those most in need.
  • Embedded Behavioral Health: Access to onsite Social Workers, Licensed Clinical Workers, Psychiatrists, and Advanced Practice Nurses for comprehensive behavioral health support.
  • Collaborative Hospitalist Support: Smooth transitions and assistance for patients requiring inpatient care.
• Patient-Centered & Accessible:
  • In-Person and Virtual Care: Provide patients with flexible options, including both onsite and full-time virtual primary care, with same or next-day appointments available seven days a week.
  • Comprehensive Services: Onsite laboratory and community pharmacy for enhanced convenience and efficiency.
• Commitment to Health Equity & Community:
  • Serve diverse populations through our FQHC locations, recognized as Patient-Centered Medical Homes (PCMH), offering sliding-scale fees and care for all, regardless of financial capability.
  • Engage in community health initiatives, wellness programs, and projects aimed at improving population health.
