Physical Therapist PT – Home Health Services - SIGN ON BONUS $15,000 AND RELOCATION $5,000 available

JOB PURPOSE:

The primary role is to deliver evaluative, consultative, and therapeutic interventions to home health patients in their residences, as referred by a licensed physician. This position requires maintaining effective communication with home health partners, patients, their families, and other relevant stakeholders.

This role provides competitive compensation of up to $125,000 annually, depending on experience, schedule, and location. Additionally, it includes a generous Stay & Earn sign-on bonus, comprehensive benefits, and a collaborative culture that emphasizes growth and excellence.

KEY RESPONSIBILITIES:
  • Assess the patient's functional level and therapy requirements.
  • Administer treatment to alleviate pain, enhance or restore function, and achieve optimal outcomes in line with the established care plan.
  • Consult with families and interdisciplinary team members to foster the best functional outcomes.
  • Evaluate, monitor, document, and report on the patient's health and condition using required documentation through preferred technology, adhering to established timelines.
  • Ensure continuity of care by collaborating with the interdisciplinary team and attending physician to develop, integrate, and manage the patient care plan, making referrals to other disciplines as necessary.
  • Exhibit intermediate knowledge of home care and demonstrate competency in specific patient care skills essential for providing care.
  • Conduct field visits to evaluate and initiate care, as well as monitor clinical outcomes and implement performance improvement initiatives.
  • Utilize approved educational materials for patient and family education.
  • Complete documentation promptly and accurately at the point of care, in accordance with industry standards (e.g., OASIS assessments, SOC/Admissions, Recertifications, ABNs, subsequent visits, physician orders, care coordination, etc.).
  • Meet agency productivity standards while effectively managing time and resources.
  • Participate in center/agency surveys (Licensure/Joint Commission), mandatory in-services, case conferences, staff meetings, and programs (e.g., Performance Improvement Program) as required.
  • Adhere to established safety regulations and procedures, reporting any job-related functions/tasks that involve occupational hazards as necessary.
  • Maintain current licensure, certification, and clinical records to comply with federal, state, and agency regulations and guidelines.
  • Recognize and respond to patient care priorities, demonstrating initiative and sound judgment.
  • Show adaptability to changes in workload and assist in training or orienting others.
  • Take responsibility for reporting any suspected violations to supervisors or relevant parties until the issue is resolved.
  • Demonstrate the ability to be self-directed, prioritize tasks, manage time effectively, and achieve outcomes and strategic goals by set deadlines.

LICENSURE, CERTIFICATION AND EDUCATION REQUIREMENTS:
  • A valid professional license in the state of practice is required.
  • Graduation from an accredited Physical Therapy Program is necessary.
  • If trained abroad, the program must be recognized as substantially equivalent to U.S. entry-level education for Physical Therapists by a credentials evaluation organization approved by the APTA.
PREFERRED SKILLS AND EXPERIENCE:
  • Experience in Home Health is preferred.
