Job Summary and Qualifications

In this role, you will deliver coordinated nursing care to patients of various age groups in their homes. You will be accountable for collaborating with an interdisciplinary team to set and achieve patient objectives while ensuring high-quality care. Your work will be guided by physician orders and supervised by the Clinical Manager.

Your responsibilities will include:

  • Evaluating home care patients to identify their physical, psychosocial, and environmental needs, as documented in clinical records, case conferences, team reports, call logs, and on-site assessments.
  • Completing OASIS assessments and visit documentation in line with agency policies, ensuring clinical notes reflect ongoing communication and coordination with physicians, team members, and patients/families/caregivers.
  • Reporting significant findings, issues, and changes to the Clinical Manager and physician, while documenting all observations, communications, and necessary interventions.
  • Overseeing and providing clinical guidance to home health aides and LPNs/LVNs to guarantee the quality and continuity of care.
  • Participating in on-call rotations and emergency calls as per agency policy.

Qualifications Required:

  • Basic Cardiac Life Support certification must be obtained within 30 days of starting employment.
  • A valid Driver's License.
  • Current Registered Nurse license.
  • Associate Degree in Nursing.
