The Role\n

As an Advanced Practice Provider (NP or PA), you will play a critical role in delivering comprehensive survivorship care for patients navigating life after cancer treatment.

\n

This is a telehealth-first position—all visits are conducted virtually—allowing you to combine flexibility with meaningful, high-touch patient care.

What You’ll Do\n

\n
• Conduct virtual survivorship visits and develop individualized, patient-centered care plans
\n
• Manage long-term and late effects of cancer treatment, including chronic conditions
\n
• Coordinate evidence-based surveillance and recurrence monitoring
\n
• Provide education, counseling, and psychosocial support via telehealth
\n
• Collaborate with a multidisciplinary care team (oncology, behavioral health, care navigation)
\n
• Help design and refine clinical workflows, SOPs, and care protocols
\n
• Contribute to quality improvement, patient experience, and program scalability
\n

Who We’re Looking For\n

• Nurse Practitioner (NP) or Physician Assistant (PA) with active MO license (or ability to obtain a MO state licenses)
\n
• Strong clinical judgment and comfort delivering care in a virtual environment
\n
• Excellent communication skills with a patient-first, empathetic approach
\n
• Ability to thrive in a fast-paced, evolving startup environment
\n
• Highly organized with strong attention to detail
\n
