Position Overview and QualificationsAs a Licensed Practical Nurse (LPN) or Licensed Vocational Nurse (LVN) within our organization, you will be essential in offering compassionate, individualized care to patients in their homes. Working alongside Registered Nurses and the care team, you will assist in managing medical needs and fostering patient independence—treating each patient as if they were family, one home visit at a time.
Your key responsibilities will include:
- Delivering nursing care at home, which encompasses wound care, medication administration, and assistance during procedures.
- Accurately documenting assessments, interventions, and outcomes to ensure safe and continuous care.
- Monitoring patient conditions and notifying the care team of any changes to facilitate timely and coordinated responses.
- Educating patients and their families to enhance medication adherence, improve understanding of health conditions, and encourage greater independence at home.
- Collaborating closely with RNs, healthcare providers, therapists, and care coordinators to provide high-quality, personalized care in a home environment.
- Basic Cardiac Life Support certification must be obtained within 15 days of starting employment.
- Valid Driver's License (DL).
- Licensed as a Practical or Vocational Nurse (LPN/LVN).
- Completion of an Associate Degree or graduation from a Vocational School.
Minimum of 1 year of relevant experience is required.
