Are you a compassionate Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) with experience in acute care? Our client is seeking a dedicated Social Work Care Manager to join a collaborative healthcare team focused on improving patient outcomes through exceptional care coordination and discharge planning.


In this role, you will partner with physicians, nurses, case managers, therapists, and community providers to ensure patients receive the support and resources they need for safe, successful transitions from the hospital.


Key Responsibilities:

  • Conduct comprehensive psychosocial assessments upon admission and throughout hospitalization.
  • Identify patients at high risk for readmission and develop individualized care transition plans.
  • Coordinate discharge planning, referrals, and post-acute services.
  • Educate patients and caregivers on discharge plans and available community resources.
  • Collaborate with interdisciplinary teams to ensure continuity of care.
  • Coordinate with insurance payers to facilitate appropriate discharge planning.
  • Participate in multidisciplinary care conferences and patient care planning.
  • Maintain accurate documentation in the electronic medical record.
  • Support initiatives focused on reducing length of stay, preventing readmissions, improving patient satisfaction, and enhancing quality outcomes.
  • Participate in quality improvement projects and departmental initiatives.


Qualifications

Required:

  • Master's Degree in Social Work (MSW) from an accredited program.
  • Current New York State LMSW or LCSW license.
  • Strong communication, organizational, and critical thinking skills.
  • Ability to work independently and collaboratively in a fast-paced healthcare environment.


Preferred:

  • Two or more years of experience in an acute care hospital or healthcare setting.
  • Experience with discharge planning, care coordination, and transitional care.
  • NYS SCREEN Certification (or willingness to obtain within the first three months of employment).
