As a Nurse Practitioner (NP) or Physician Assistant (PA) within our Senior Primary Care division, you will collaborate with a diverse team that includes physicians, pharmacists, care coaches, and behavioral health experts, all committed to delivering tailored, high-quality care.
In this position, you will serve as a float Advanced Practice Provider (APP) across clinics in the San Antonio area, reporting directly to the Associate Medical Director.

Your Responsibilities
- Deliver comprehensive care to seniors in accordance with established clinical standards.
- Engage in daily collaboration with your care team to ensure seamless, high-quality service.
- Oversee referrals, coordinate with hospitals and skilled nursing facilities, and manage durable medical equipment and home health services.
- Accurately document patient care with assistance from quality-focused coders.
- Participate in on-call rotations and contribute to the growth initiatives at your center.
- Demonstrate a commitment to providing value-based care.
Qualifications We Seek
Essential:
- A Master’s Degree in Nursing or completion of a PA program with board certification.
- An active, unrestricted NP or PA license in the state where you will practice.
- Successful candidates will undergo TB screening if selected for this position.
Preferred:
- Two or more years of experience in value-based care, primary care, or Medicare Advantage environments.
- An active DEA license and Medicare/Medicaid provider numbers, or the ability to obtain them.
- Bilingual proficiency in Spanish and English.

Compensation Range
The salary range provided below represents a good faith estimate of the starting base pay for full-time (40 hours per week) employment at the time of posting. Actual compensation may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
$123,800 - $170,300 per year
This position is eligible for a bonus incentive plan based on individual and/or company performance.
