\nYou will provide person-centred support to individuals with learning disabilities, autism, mental health needs, or health conditions across different care settings.
\nShift Patterns
\nThis role may include a mix of:

- Day shifts \n
- Late shifts \n
- Sleep-in shifts \n
- Waking night shifts \n
- Every other weekend working \n
Flexibility is essential to meet the needs of the service.
\nKey Responsibilities

- Assisting with personal care, daily living tasks, and health needs \n
- Promoting independence, choice, and dignity \n
- Supporting individuals to access the community and achieve personal goals \n
- Following care plans, risk assessments, and clinical guidance \n
