Job Description
Key Responsibilities

\n - Carry out holistic assessments of children and young people with disabilities in their home environments.
\n - Asses, recommend and provide specialist equipment to support daily living and independence i.e. specialist seating, moving and handling, bathing and toileting.
\n - Assess for and contribute to home adaptations, ensuring environments are safe, functional, and fit for purpose.
\n - Work closely with families and carers, providing guidance, reassurance, and practical support.
\n - Collaborate with multi-disciplinary professionals across
\n - Council services
\n - Health (e.g., NHS colleagues)
\n - Education providers
\n - Partner and voluntary agencies
\n - Contribute to care planning, ensuring interventions are child-centred and outcome-focused.
