Under the guidance of the medical director, the Primary Care Physician (PCP) delivers essential medical services to participants of the PACE Organization. This role encompasses evaluating patient needs, collaborating with an interdisciplinary team to formulate care plans, providing direct medical attention, and assessing the effectiveness of these plans. The PCP also engages collegially with other healthcare professionals, making autonomous decisions regarding medical requirements and health strategies, and performing necessary medical procedures.

Key Responsibilities:
  • Actively participate as a member of the interdisciplinary team (IDT) during initial, semi-annual, unscheduled, and annual evaluations; attend daily morning updates and communicate any changes in participants' baseline conditions to relevant staff.
  • Conduct initial and ongoing assessments of participants, develop care plans, and implement these plans. Ensure that the primary care treatment strategy aligns with the overall care plan established by the interdisciplinary team.
  • Refer participants to medical specialists as needed. Oversee the care of participants in nursing facilities, conducting regular visits in collaboration with the IDT, maintaining communication with nursing home staff for continuous monitoring, and facilitating hospital admissions when necessary.
  • Manage and oversee participants' interactions with medical specialists and inpatient care, ensuring effective communication and follow-up regarding the outcomes of specialist visits.
  • Engage as an integral member of the interdisciplinary team, attending meetings regularly, sharing updates on participants, and collaborating on care planning to ensure 24-hour care delivery.
  • Review medical outcomes, statistical data, and utilization metrics under the direction of the Medical Director, identifying issues such as unnecessary hospitalizations and working with the Medical Director to implement best practices and procedural changes to enhance participant health.
  • Complete necessary documentation and utilize appropriate diagnostic coding.
  • Advise the Medical Director on strategies to improve accountability of services to participants and referral sources, while keeping the Medical Director informed about resource needs as the program grows.
  • Assist in developing policies, procedures, and standards of care; continuously monitor and evaluate patient care practices and service delivery; provide guidance and training to staff on medical and quality assurance matters.
Minimum Qualifications:
  • Doctorate Degree (MD or DO) with a current and active physician license to practice in the state required. CDS Certification and DEA Registration are mandatory. Current BLS certification is also required.
  • Board Certification in Internal Medicine or Family Practice is essential; Board Certification in Geriatrics is preferred.
  • A minimum of one (1) year of experience working with elderly or frail populations is required.
  • A valid driver's license, proof of insurance, and reliable transportation are necessary.
  • Must be proactive, possess business acumen, manage conflicts effectively, focus on customer service, demonstrate high-quality decision-making, adapt to change, and exhibit organizational agility. Ability to work independently and exercise sound judgment is essential.
  • Strong interpersonal skills to foster collaboration, commitment, and productivity when working with diverse teams, customers, and end users. Comfort in a virtual, collaborative leadership environment is important.
  • Exceptional written and verbal communication skills tailored to audience comprehension, with the ability to engage effectively with individuals and groups from various backgrounds.
  • Proficient in the Microsoft Office suite (Word, Excel, PowerPoint, etc.) and capable of using other software necessary for job functions.
  • Excellent organizational abilities, capable of managing multiple tasks related to daily operations while maintaining high customer service standards. Ability to handle frequent interruptions and adapt to changing priorities is crucial. Strong problem-solving skills are required.
  • Critical thinking skills and the ability to prioritize workload effectively are essential.
  • This position requires occasional travel within the geographic region of the organization.
