Specialty Primary Care IM/FM trained MD/DO accepted Coverage needs: ASAP Ongoing Shifts: Monday Friday 8a-5p Setting: Outpatient facility Value Based Care model Seeing ages 65+ Average patients per day: 10-13 Patient panel max of 400 Support staff: Each provider has their own care team/scheduler, MA, referral specialist, Nurse, Case Manager, Social Worker and Outreach Specialist EKGs and labs are performed in center EMR: Epic Travel accommodations Covered by facility Credentialing timeline: 2-3 weeks Requirements: Active AL license Active AL DEA Active BC
