Position: Home Health Physical TherapistIn this role, your responsibilities will include:
- Planning and delivering prescribed physical therapy treatments to patients in their homes, aiming for the highest possible level of physical function.
- Assessing and documenting patients' physical capabilities to assist in physician evaluations and inform treatment objectives.
- Creating and executing rehabilitation programs that align with the physician's treatment plan and the goals set by the patient care team.
- Providing clear written care instructions to Physical Therapy Assistants (PTAs) and Home Health Aides, while supervising their performance and documentation.
- Promptly and thoroughly documenting patient observations, interventions, and evaluations.
- Communicating patient progress to the physician, Clinical Manager, care team, patient, and family, while submitting all necessary evaluations, treatment plans, progress notes, and discharge summaries.
- Degree from an accredited Physical Therapy Program recognized by the APTA.
- Current and unrestricted Physical Therapy license.
- Valid CPR certification.
- Experience working collaboratively with a team of healthcare professionals.
- At least one year of experience as a Physical Therapist.
• Pay per visit/unit: $59.00 - $83.00
• Annual base salary: $93,000 - $128,000
Additional InformationTuberculosis Screening:This position involves direct patient interaction and is part of our Tuberculosis (TB) screening program. Candidates selected for this role will need to undergo TB screening.
Driving Requirements:This role is included in our driver safety program, necessitating a valid state driver’s license. You are expected to maintain personal vehicle liability insurance, meeting or exceeding state minimum requirements, or $25,000 for bodily injury per person, $25,000 for bodily injury per event, and $10,000 for property damage, whichever is greater.

Compensation RangeThe salary range provided reflects a good faith estimate of the starting base pay for full-time employment (40 hours per week) at the time of posting. Actual pay may vary based on geographic location and individual qualifications, including skills, experience, education, and certifications.
