The Role:\n

Were seeking a Nurse Practitioner Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, youll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention. Your clinical expertise and patient-focused approach will support our mission of delivering high-quality primary care in a compassionate and efficient environment.

What Youll Do:\n
    \n
  • Perform comprehensive physical examinations and health assessments.
  • \n
  • Diagnose and manage acute and chronic illnesses.
  • \n
  • Order, interpret, and evaluate diagnostic tests, including labs and imaging.
  • \n
  • Develop and implement individualized treatment plans.
  • \n
  • Prescribe medications and therapies in accordance with state regulations.
  • \n
  • Provide preventive care, including immunizations and routine screenings.
  • \n
  • Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease.
  • \n
  • Educate patients and families on disease management, lifestyle modifications, and preventive care.
  • \n
  • Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies.
  • \n
  • Coordinate care with specialists and other healthcare professionals.
  • \n
  • Document patient encounters accurately and timely in the electronic health record (EHR).
  • \n
  • Participate in quality improvement initiatives and population health management.
  • \n
  • Other relevant duties as assigned by management.
  • \n
What Were Looking For:\n
    \n
  • Masters or Doctorate degree in Nursing (MSN or DNP).
  • \n
  • Graduate of an accredited Nurse Practitioner program.
  • \n
  • Current Registered Nurse (RN) license.
  • \n
  • Current Nurse Practitioner license in the practicing state.
  • \n
  • National board certification (AANP or ANCC) Family NP or Adult-Gerontology NP.
  • \n
  • DEA registration (if prescribing controlled substances).
  • \n
  • BLS/ACLS may be required depending on setting.
  • \n
  • Strong clinical assessment and diagnostic skills.
  • \n
  • Excellent communication and patient education abilities.
  • \n
  • Knowledge of evidence-based practice guidelines.
  • \n
  • Ability to work independently and collaboratively.
  • \n
  • Proficiency with electronic medical records (EHR/EMR).
  • \n
  • Cultural competency and a patient-centered care focus.
  • \n
Work Environment:\n
    \n
  • Outpatient primary care clinic.
  • \n
  • May include community health centers, private practices, or hospital-affiliated clinics.
  • \n
  • Typical schedule: MondayFriday, 8am5pm.
