Physical Therapist (PT)

Job Type: Full-time, PRN

Salary Range: $45-$55/hr based on experience

Our facilities function as Skilled Nursing Facilities (SNFs) that offer:

• Long-term care for residents needing continuous skilled assistance and compassionate daily support.
• Short-term, sub-acute rehabilitation for individuals recovering from surgery, illness, or injury.
• Collaborative care through a team-oriented approach involving nursing, therapy, and medical professionals working in unison.
• A varied patient demographic including those with orthopedic, neurological, cardiopulmonary, and post-acute requirements.

The patient mix may differ slightly across facilities. As a therapy professional, your role will be to assist patients in achieving functional improvements and enhancing their quality of life in a supportive, patient-focused setting.

Position Overview

The Physical Therapist (PT) delivers direct rehabilitation services in physical therapy to patients referred by physicians, adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensing Criteria

• Graduation from an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT).
• Current registration/licensure in the relevant state, with an obligation to maintain an active license in good standing throughout employment.
• Preferred one (1) year of experience in a post-acute care or similar environment.
• CPR certification required upon hiring or to be obtained during orientation; certification must remain current throughout employment.

Specific Job Responsibilities

• Engage in community health initiatives/projects as appropriate.
• Proficient in Microsoft Word, Excel, and email communication.
• Collaborate with patients, families, and support departments to effectively address patient needs.
• Consistently demonstrate proper body mechanics.
• Make independent decisions when necessary.
• Familiarity with practices, procedures, and regulations governing operations in a post-acute care setting.
• Implement and interpret departmental programs, goals, objectives, policies, and procedures.
• Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation.
• Maintain professional relationships with colleagues, vendors, and others.
• Ensure confidentiality of all proprietary and sensitive information.
• Understand and adhere to organizational policies, including those related to harassment and compliance.
• Demonstrate integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training.

Essential Duties

• Implement physical therapy treatment plans for patients.
• Set, evaluate, and adjust realistic, measurable, timely, and functional goals.
• Supervise and assess care provided by PTAs, Rehab Aides, and PT and PTA students.
• Document care accurately and in a timely manner.
• Utilize therapy software effectively and accurately.
• Provide excellent customer service and maintain a positive attitude towards patients.
• Assist in the safe evacuation of patients when necessary.
• Exhibit reliable and consistent attendance.
• Employ reasoning skills and sound judgment in all situations.
• Communicate effectively and work collaboratively within an interdisciplinary team.
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently throughout the workday.
• Read, write, speak, and comprehend the English language.
