Job Type: Full-time, PRN

Wage Range: $45-$55/hr based on experience

Setting & Population Served

Our facilities operate as Skilled Nursing Facilities (SNFs) that offer:

• Long-term care for residents needing ongoing skilled support and compassionate daily assistance
• Short-term, sub-acute rehabilitation for patients recovering from surgery, illness, or injury
• Collaborative care through an interdisciplinary team approach involving nursing, therapy, and medical staff
• A diverse patient population including individuals with orthopedic, neurological, cardiopulmonary, and post-acute needs

The patient mix may vary slightly by facility. As a therapy professional, you will assist patients in achieving functional progress and enhancing their quality of life in a supportive, patient-centered environment.

Position Summary

The Physical Therapist (PT) delivers direct rehabilitation care in physical therapy to patients based on physician referrals, adhering to all relevant laws, regulations, and organizational standards.

Education, Experience, and Licensure Requirements

• Graduation from an accredited physical therapy program, holding a BSPT, MSPT, or DPT (those graduating in 2003 or later must possess an MSPT or DPT)
• Current registration/licensure in the applicable state, with an obligation to maintain an active license in good standing throughout employment
• Preferred one (1) year of experience; new graduates are encouraged to apply
• CPR certification required upon hire or to be obtained during orientation, with the necessity to keep it current during employment

Specific Job Requirements

• Engage in community health initiatives/projects as appropriate
• Proficient in Microsoft Word, Excel, and email communication
• Collaborate with patients, families, and support departments to effectively plan for patient needs
• Consistently demonstrate proper body mechanics
• Make independent decisions when necessary
• Familiar with practices, procedures, and regulations governing operations in a post-acute care facility
• Implement and interpret departmental programs, goals, objectives, policies, and procedures
• Exhibit proficiency in all competency areas, including patient rights, safety, and sanitation
• Maintain professional relationships with all associates and vendors
• Ensure confidentiality of all proprietary and confidential information
• Understand and adhere to company policies, including those related to harassment and compliance
• Exhibit integrity and professionalism by following the organization’s Code of Conduct and completing mandatory compliance training

Essential Functions

• Implement physical therapy treatment plans for patients
• Set, assess, and adjust realistic, measurable, timely, and functional goals
• Supervise and evaluate care provided by PTAs, Rehab Aides, and PT and PTA students
• Document care accurately and in a timely manner
• Utilize therapy software effectively and accurately
• Provide excellent customer service and maintain a positive attitude towards patients
• Assist in the evacuation of patients when necessary
• Demonstrate reliable and regular attendance
• Employ reasoning skills and sound judgment
• Communicate effectively and collaborate productively within an interdisciplinary team
• Engage in physical activities such as sitting, standing, bending, lifting, pushing, pulling, stooping, walking, reaching, and moving intermittently during work hours
• Read, write, speak, and comprehend the English language
