Community RN Career Direct Care Pathway Placement

The Community RN Career Direct Care Pathway consists of four distinct levels, determined by a Registered Nurse's (RN) experience, education, and qualifications:

  • Level I: Less than 12 months of RN experience.
  • Level II: 12 months to less than 3 years of RN experience.
  • Level III: 3 years to less than 8 years of RN experience.
  • Level IV: 8 years or more of RN experience.
Job Overview

The RN Complex Care is responsible for delivering direct nursing care to patients and their families, adhering to established policies, procedures, and practices within a complex care environment.

Location: Main Campus
Unit: ICU-1
Shift: 36 hours, Evenings | Nights 7:00 PM - 7:00 AM
Weekends and Holidays: Required

Unit Summary

ICU-1 is a 12-bed Intensive Care Unit that caters to both medical and surgical critically ill patients. This includes those with cardiac, pulmonary, neurological, hematologic, obstetric, gastrointestinal, renal, vascular, and multi-system conditions. Patients may require hemodynamic or respiratory support, close monitoring, or intensive nursing care, including interventions such as IABP, Impella devices, lysis, EKOS, TPA, hemodialysis, CRRT, PA lines, or 24-hour EEG monitoring.

Staff in the ICU benefit from ongoing training, both at the bedside and in the classroom. Newly hired nurses participate in AACN’s online training program, ECCO, as part of their onboarding process. Our commitment is to provide high-quality care to every patient in the ICU.

Job DescriptionMinimum Qualifications
  • Valid Massachusetts RN Licensure.
  • Current Basic Life Support (BLS) Certification.
  • Department-specific certifications may be required based on the type of patient care provided.
  • Experience level must align with one of the specified levels (I, II, III, or IV).
Preferred Qualifications
  • Bachelor of Science in Nursing (BSN).
Duties and Responsibilities

The following duties and responsibilities outline the general nature of the work and are not exhaustive. Additional tasks may be assigned as needed:

  • Engage in and maintain awareness of performance improvement initiatives.
  • Evaluate and incorporate evidence-based practices and research into clinical care.
  • Conduct and accurately document clinical assessments.
  • Develop, implement, and assess individualized patient care plans.
  • Utilize professional nursing judgment to tailor care plans based on patient assessments.
  • Manage patient assignments by delegating tasks effectively according to patient needs and team member skills.
  • Create comprehensive educational plans for patients and families, utilizing appropriate resources and documenting per departmental standards.
  • Support patient and family rights by facilitating informed decisions and respecting advanced directives.
  • Continuously assess and update care plans based on changes in patient health status.
  • Formulate comprehensive discharge plans, incorporating relevant resources and referrals.
  • Assist in the development of new staff members and support their integration into the unit.
  • Guide the professional development of department staff through precepting.
  • Engage in professional growth by identifying learning needs and pursuing relevant education.
  • Participate in departmental committees as required.
Physical Requirements
  • Occasional lifting of 30-35 lbs and repositioning of patients.
  • Prolonged periods of standing and walking.
  • Occasional twisting, bending, reaching, pushing/pulling, sitting, kneeling, and squatting.
  • Exposure to infectious diseases and potential carcinogenic substances.
  • Risk of exposure to blood-borne pathogens.
  • Fine manual dexterity is required for patient handling.
  • Visual and auditory acuity necessary for monitoring patient care.
Skills & Abilities
  • Effective communication skills with patients, families, physicians, and the healthcare team.
  • Knowledge and application of the nursing process.
