Paediatric Nurse Care Co-ordinator (Band 5)

About the Role

We are seeking a compassionate and organised Care Co-ordinator to join an ambitious and growing service supporting children with complex, serious or life-limiting conditions. This is a rewarding opportunity to make a meaningful difference, working closely with children and their families both in hospital and at home.

You will play a vital role in coordinating care, supporting hospital discharges, and ensuring smooth transitions between clinical and community settings. Working as part of a collaborative multi-disciplinary network, you will help deliver high-quality, person-centred support to those who need it most.

Key Responsibilities

• Manage a caseload of children and families, providing tailored support both in hospital and at home
\n
• Work closely with hospital discharge teams to enable safe and timely transitions from hospital to home
\n
• Act as a key liaison between hospital teams, community services, and families to ensure seamless care coordination
\n
• Support children and families emotionally and practically through complex and challenging situations
\n
• Maintain accurate records, reporting, and data management to a high standard
\n
• Build effective partnerships with healthcare professionals and stakeholders
\n
• Contribute to continuous service development and improvement
\n

Essential Skills & Experience

• Experience working with children and families with complex care needs in hospital and/or community settings
\n
• Strong communication and interpersonal skills, with the ability to build trust and rapport
\n
• Ability to work collaboratively with multidisciplinary teams
\n
• Excellent organisational skills, with the ability to manage competing priorities effectively
\n
• Confident using IT systems, including databases and reporting tools
\n
