- Carry out comprehensive assessments to identify the practical, emotional and safety needs of survivors and their children. \n
- Develop and review personalised support plans and risk management plans through regular key work sessions. \n
- Provide ongoing emotional support, advocacy and practical assistance to help individuals achieve their goals. \n
- Support clients to access safe accommodation, healthcare, benefits, legal advice, education, employment and other relevant services. \n
- Work collaboratively with local authorities, housing providers, health professionals, social care, police and other partner agencies. \n
- Safeguard adults and children by identifying risks, following safeguarding procedures and escalating concerns appropriately. \n
- Maintain accurate and confidential case records using internal case management systems. \n
- Contribute to positive outcomes by empowering survivors to increase their independence, confidence and resilience. \n
- Participate in an out-of-hours on-call rota and travel across the service area when required. \n
- \n
- Experience working within domestic abuse, housing, homelessness, social care, mental health or a related support service. \n
- Good understanding of the dynamics and impact of domestic abuse on survivors and children. \n
- Experience carrying out assessments, developing support plans and managing complex caseloads. \n
- Knowledge of safeguarding procedures and trauma-informed approaches. \n
- Excellent communication, advocacy and relationship-building skills. \n
- Ability to work collaboratively with a range of statutory and voluntary agencies. \n
- Strong organisational skills and experience maintaining accurate case records. \n
- Full UK driving licence and access to your own vehicle for work purposes. \n
- Flexible to participate in an out-of-hours on-call rota. \n
