Schedule: Monday-Friday | 8:00 AM - 5:00 PM

Position Overview

A leading healthcare organization is seeking a Case Management Assistant II to support its Care Coordination team at a San Francisco hospital campus. This is a primarily administrative, non-clinical role focused on coordinating skilled nursing facility admissions, discharge planning, and transition-of-care activities. The ideal candidate is highly organized, communicates effectively with multiple stakeholders, and thrives in a fast-paced healthcare environment.

Responsibilities
  • Support discharge planning and transition-of-care processes for patients moving to skilled nursing and post-acute care facilities.
  • Coordinate with healthcare providers, facilities, families, insurance representatives, and external vendors to facilitate timely patient placement.
  • Gather, organize, and maintain required documentation to support admissions and discharge workflows.
  • Track multiple cases simultaneously while ensuring accuracy and timely follow-up.
  • Maintain electronic records and documentation in accordance with departmental procedures.
  • Communicate updates and escalate questions or issues to appropriate team members when needed.
  • Assist the Care Coordination department with administrative and operational support as required.
Qualifications
  • High school diploma, GED, or equivalent required.
  • At least 1 year of recent experience in healthcare operations, care coordination, case management support, admissions, discharge planning, or a related administrative healthcare role.
  • Strong organizational skills with the ability to manage competing priorities.
  • Proficiency with Microsoft Office, electronic documentation systems, and data entry.
  • Excellent verbal and written communication skills.
  • Ability to work independently while collaborating effectively with multidisciplinary teams.
  • Skilled nursing facility admissions, intake, or discharge planning.
  • Skilled nursing, rehabilitation, post-acute care, or hospital environments.
  • Insurance verification, payer coordination, or authorization processes.
  • Communicating with healthcare providers, patients, families, and community resources.

Pay Details: $25.00 per hour
